He swung the axe with one hand (in another direction) and punched his friend with the other?

It only says he hit Timmy, but never says what with.

Was the axe a toy axe or was Tommy a toy?

No.

the axe didn't hit his neck?

It did.

He didn't swing the axe AT Tommy. Maybe he swung it at a log, but a chip of wood flung up and hit Tommy in the neck?

He swung and hit him with the handle end? Or he swung very lightly as a joke, cos he didn't want to kill his friend.

Nope.

Is the axe an actual woodcutting axe? No.

Is the neck an actual human neck? Yes.

Was the axe a guitar?

Correct!

An axe is a common nickname for a guitar; when Matthew swung an 'axe', he was actually swinging his guitar and hit Tommy in the neck. Luckily for Tommy he was just hit by the body of guitar, thus he suffered no serious injuries.

Congratulations! Your turn!
