There are 2 kinds of serum you can use on NPCs to make them talk to you: serum 207 (tarromin + ashes) only has a temporary effect, and serum 208 (made by using serup 207 on the fire in temple) has a permanent effect. You have an option of either using a 207 on the shop keeper each time you want to trade, or make a 208 one time in order to get a perma access to the shop.
