Nothing. The only reason all items have an operate function when equiped is

 

 

 

that it's impossible for JaGEx to specify each individual item that uses the operate function, so they've added it to all items.

 

 

 

When you try to operate it you simply get the message: There's no way to operate that item.

Anything that doesn't have an effect (teleport for an example) does nothing. It just says "you cannot operate this item" in your chatbox.
