Yes, you start pixel by pixel. Don't feel bad if your first works look blocky or squarish. In your graphics program it's possible that you have an option to increase the pixels per inch which will make it look smoother. The standard is usually 72 pixels per inch, try a blank canvas with, for example, 300 pix/inch and work from there.
