There is a misleading picture in the guide, under 6: the same, but different.

 

 

 

the picture shows the puzzle completed with a combination of 1, 2, and 2 discs where you actually need to do 1, 2, and 3 disks.

 

 

 

i took a screen shot of a better image for the guide.

also, the guide, under 4: Fixing the Machine,

 

it mentions the use of "normal trees with pointed tops" in place of evergreens because of classification, but when i looked around the stronghold i couldn't figure out which trees they were referring too. Perhaps someone who knows better could take a screenshot / make a map to where these trees are?

You can get extra crystal disks from the chest in Golrie's cell under the treegnome village i have screen-shots but don't know how to upload them yet :)

Thank you the OP's suggestions will appear in the guide shortly.

 

At this time I will not be adding the note about extra discs as it is a lot of work to head all the way to Tree Gnome Village when the exchanger is right there.
