After playing with my prayers for the past few week against this guy I can 100% confirm

That ONLY the first and 2nd forms have mage attacks.

As soon as it hits the third form it has only melee attacks left.

Edit: I have edited the Bestiary entrys on the 3rd and 4th forms as well as the quest guide and you have been credited in all those places good job! :thumbsup:
