The end of the guide currently states that a slashed book, two body bars, and a hammer are required to create a body body.

This is incorrect, there is one more item required: the ragged book used to start the quest. You need this as well as your slashed book in order to create the body body.

I thought I'd finish the quest, then submit the error report. I didn't realize at that point you also wished for screenshot proof so all I have to offer is my chat log notifications, for which I apologize. At any rate though, I'm sure it won't be too difficult to get confirmation for this if my proof is insufficient.
