What is it used for and is there anything good to put in that slot?

Can you include a picture of your setup? Depending on what size the interface is and other things, which slot is to the left of cape can vary, but with a picture I should be able to identify it.

 

If on the off chance you mean the icon with the swirl type symbol in it, those refer to sigils which can be earned from an activity called Shattered Worlds. They convey some small benefits for doing certain kinds of Slayer, but you can read more about them on that page.

 

Edit: appears there are a few more useful ones from Menaphos I was not first aware of. This page is a better reference.
