Erm, how much are you exactly expecting for this? Cause, it looks 'ok' I guess, but that can be made very quickly and easily, and contains hardly any effort :?

 

 

 

Bit too simple/plaing/boring, try adding something, and lower the contrast between the text field/box and the background...
