What's the most cost effective way to do smithing from 58-70?

Fastest would be steel plates. Smelting steel bars is another possibility if you want to make some nice cash, but is very slow.

Depends on if you are a member or non member. I would say if you are a member, play Stealing creation and get enough morphic hammers and use steel bars. In f2p anything other than superheating is not worth it.

Cannonballs, but it's not very fast.

There is no way to make "Millions fast and easy"...
