Its true that you can get a drop worth almost 20m but it takes thousands of kills and even then your not sure to get one. Fire Bolt with Chaos Gauntlets seems to be the best thou. As for breaking even without a visage; it seems likely you will profit by a small bit, since you'll get rune and dragon items from time to time.
