I had my first two attempts at fight caves for the first time and failed both after the healers appeared. Pray switching wasn't a big problem, but the healers I distracted continued to heal Jad, which I found out was new thing after the graphical update (probably switched prayer long enough to kill Jad 4 times over.....

 

If I kill off all the healers, would more healers spawn? :unsure:

Yea they respawn. I have heard, although not tested myself, that it is possible to kill Jad without killing the healers if you have a good ranged level and since you have 99 it shouldn't be too much of an issue for you.

Do the healers always come in the same direction?

The healers will respawn if Jad gets back to full health, but if you use deflect curses this shouldn't be a problem at all.
