Either put the cat you already have in your house's Menagerie(or build one if you don't), shoo it away, or sell it to the person in Ardougne(can't remember name) who buys cats for 200 death runes.

 

You can't get another cat/kitten until you do one of these things.

Just to clarify you CAN get a new kitten, but only when your existing cat is no longer growing and in-need of care. This means when it is overgrown, lazy or wily; while it is still a kitten or a plain cat you cannot get a new one.
