I saw this while I was in Varrock today and I don't think I've ever seen one before. What is it?

When POHs came out, you could make all kinds of shields with patterns on them. That's a skull shield with either steel or rune stats.

No longer playing Runescape, I caught the WoW bug.

Ah, well that would explain why I didn't know what it was. I quit for a while just before POHs came out and just started yesterday...
