How do I start a game of FOG 1-1?
Does the rating have to be exactly the same - I'm 241 & a friend is 229 but we couldn't start !

You can't. You need to have a certain number of people, I think it's 20 but I'm not sure. Try world 115 if you're looking to boost (hunter forfeits).

Just legit it. Familars are not effected by the accuracy nerf, so your steel titan could very well kill your low level attackers easily till they rage quit or you get 3k+ charges. Or you could just 6-7 hit everyone and purposely die when you have more charges than them.

I did exactly that - my Steel Titan was killing the lower levels while I just stood in the middle, with them splashing on me. I did feel a bit sorry for them in the end
