You need to do Biohazard I think.

During Plague city quest, you can enter west ardy through an underground sewer which gets blocked at the end of the quest.

 

 

 

To access the city again, do biohazard and through the course of the quest, 2 npcs near the south eastern wall of west ardougne will help you get in.

 

 

 

After the biohazard quest, you can walk in through the large doors openly.

Strangely with WotLK so near, I wished I could delay it a bit to push through that last TBC content in MH/BT :'(.
