Can I build a quest hall/skill hall in the dungeon and build a staircase to the ground level, or do I need a special dungeon entrance?

i think its possible, usually after you build the dungeon it all connects but im not sure bout the dungeon entrance, either that you need or a throne room. then again you gotta have a throne room for lever.

Yes. I have that kind of dungeon entrance. BTW, I think that you need spiral staircase if you want to make staircase that can go both up and down. (I have 3 skill halls one above other).

You can have dungeons w/o any entrance at all!

your dungeon will still be available even if you remove all entrances, just getting in might be more difficult. One possible (and fun way) is to build a dungeon accessible only by the pitfall trap.

As for entrances, any stairs downward will work, although spiral will allow you to have stairs going both up and down.
