September 5, 200916 yr Otto will tell you to go to a harpoon fishing spot and fish with your hand. Run to Catherby or anywhere you can fish for tuna/swordfish/shark and just click on the harpoon spot. Don't carry a harpoon with you because you will default to using it.

September 5, 200916 yr Level 55 is the minimum, I think. Have a look at this guide http://tip.it/runescape/?page=barbarian_training.htm#fish2. In reading it, it seems you only need to do the firemaking parts to access the cavern.
