This form when submitted just shows a blank page so I'm really unsure if the form submitted correctly. I've tried it about 5 times on separate occasions and from work and home without success. Is it just my bad luck or are others having problems?

 

I'd love to get this form fixed so I can submit a request through it lol.

I believe it isn't a problem on your end and that it's still being worked on. Wait for an admin to confirm that though please.
