I have encountered "harassers" in many different situations from training at moss giants to mining in the mining guild. They just start conflicts for no good reason and often times you cant report them because they are careful not to use reportable language, but then they keep provoking you enough so that you may use profanity and then they report you for offensive language. Just yesterday I was training magic at the moss giants and there was a lvl 73 and 74 there, and i am lvl 87. So the lvl 73 asks me for a law rune and i say no, while the lvl 74 is constantly calling me noob because im using fire strike, pretty soon they both start ganging up on me constantly making nonsensical statements about me and just plain being offensive but not using language that i can report them for. Now i realize the ignore list is there for a reason, but I feel that people like those i described are making the game less enjoyable and should be brought to the attention of jagex. Just for being mean, you'd get shouts of "I'm reporting you for harrassment!!!" in every single possible situation, and I'd imagine those kind of reports would flood cs's reports they have to deal with.
