Technically it would be 22 attack. And yes, HellsBells your right. But strength ammies are used more than 50% of the time, buy you can wear amulet of power which would make it -4 from str ammy but it helps prayer.

Well technically it's better to have strength amulet because I think it can make your max +1 or 2. But it gives it out that your range/melee

I don't know Hellsbell, but I'm pretty sure its like you just make it. For example: Hit 12 they on 25 hp use 2h you just hit 25.
