It was the a big bug abuse.

 

When player owned houses first came out there was a bug that, when you got kicked out off a house (note when you where kicked you where in a combat ring) the attack option remained and allowed you to attack players which where not in the wildy.

 

As most player owned houses where at rimmington at the time the people who were killed where many in fally.

There is also a rumour that people were able to attack players outside of the wilderness a few weeks ago, in Falador. I cannot find anything to confirm this, and I've dismissed it as rumour, although I met a player who told me he was there.
