Glitch?This happened to me too. As far as i'm aware there are two types of 0 in runescape. There is a complete miss which results in a 0 or there is a successful hit which hits only 0. In Kuradals dungeon all successful hits are boosted +40 (was 4) which means that it's possible to hit just a 40 in her dungeon. If the game decides on 0.1 - 0.9 you would hit 0, but it was a successfully hit and thus got boosted.
