I first started to hear about this happening soon after the release of as a first resort, when someone was saying that there iron knives were poisoning a monster when they had a pet out. Later, another person stated that the same thing happened, again with iron knives and a pet(but a different pet). I think it has something to do with throwing weapons combined with pets/familiars.
