Maybe when you Ice Barrage someone and drop something while the ice-cube thing is around you, it falls on top of it ? It's far fetched, but it's the only explenation I can come up with lol. At the Kalphites ... mmh, I guess the soldier moved towards the stalagmite when it died and the drop moved a square too, ending up there. Nothing special really, it just happens when your cannon kills 2 of the assigned slayer monsters at the same time, and when that happens to be your last monster. I was once at the airport with my friends in California, had to go "potty" really bad, and just pissed my pants in the middle of the terminal.
