Actually proper screaming is very difficult. You're right, anyone can scream, but if they don't know how to they'd destroy their lungs within days. And they'd sound like [cabbage] in the first place.

The acting required to do screamo takes very little, but good acting takes talent.

 

Example.

Lord of the Rings Two Towers.

In the movie there is a scene where Legolas, Gimli, and Aragorn some across the pile of burnt uruk-hai and think merry and pippin are dead. Aragorn, in his rage punts a nearby helmet and lets out a scream of anguish. What you didn't know is that when Viggo Mortensen(guy who played Aragorn) did that, he broke three of his toes AND KEPT ACTING. Yeah, he took the pain of three broken toes and put it all into performance.

 

Also, Metal where they scream so you can't understand them, yeah, thats about it.

Really good acting takes talent takes talent. Just acting doesn't.
