Now you ask yourself, where does he needs help with? It's a bad link, you're doesn't contain the survey's actual id string or w/e it's called in the url. There should be a long string of numbers/letters before the ? and a=IF. I would try Survey Monkey, it's a popular web based surgery service here in the US. Until you do provide a proper link that will work, I'm going to lock this, as there is no discussion value to be had.
