Some hire people, others do it themselves. Steve Harris drew up the Iron Maiden font, and I think the dude from Death did the logo for his band as well.

The only death metal band I really like is Death. I really like the genre, I just don't really know any big names in it. I plan on getting some more Death, and I'll look into some of the youtube links posted. ;P

I think the first synth stuff used was on the song Xanadu off of A Farewell to Kings, but the age of it most likely started with Moving Pictures.
