I have found Hellhounds (Member's Dungeon) to be fairly regular droppers of lvl3 scrolls - about 20ish kills per scroll or less sometimes.

I think you mean the Law of Conservation of Energy. Obviously what's happening is that the energy you expend when hammering the bars is not all used up by deforming them into platebodies. The excess is being transformed into steel - jagex must have put this feature in to stop your hammer and anvil from melting!!
