Very rare. It is ever so slightly different between elite and hard, but they are both so rare you are unlikely to notice the difference. I doubt I'd get away with giving you the exact numbers, but to give you an idea, the chance is in the 6 digits. Not quite confirmed, moreso estimated.

I always wear a RoW when I do clues and I've never gotten an item worth more than 4.5M. 99 HP, Attack, Strength, Defence, Summoning, Ranged, Herblore, Prayer, Agility, Magic, Slayer, Fletching, Fishing, Woodcutting, Mining, and Thieving. Jagex'd out of my untrimmed hp cape on 6/14/2011.
