This summer my right hand really started to hurt when playing RuneScape. I was training Prayer in F2P and was aproximately clicking 4K+ an hour to bury all those bones. I got a new mouse now and decided not to train Prayer for longer than one hour, and that really helped alot. I don't feel any pain in my hand anymore :).

I get a strain in my wrist occasionally if I'm doing a strenuous activity. It might not be RSI strictly, just muscle fatigue perhaps. I used to play RuneScape on a laptop's mousepad, so that definitely made it worse because my shoulders were cramped over the keypad as well, causing spinal strains. I just reorganised my desk, bought a USB keyboard plus a mouse and it's improved a lot. I've never played RS for long periods of time though, three hours per session at the very most followed by a break.

The really horrible thing is when I put my hands in a rested position. You can really see the curve in my fingers where a mouse/controller would go.
