As soon as they see you they know whether you are pking or something else (green slaying, clue scrolls, etc.) anyway. This would just prevent accidental attacks for non-pkers. If pkers start wearing them, it still serves the same purpose; you won't accidently skull on them. Just wear a cape and watch your back like normal.
