I would have to say the Barbarian Assault world

I would say Agility or Cooking

My top 3 most respected are Slayer, Agility, and Mining

I take that into consideration whenever I do something that could get me killed, but I always take my best stuff everytime. If I die, I die.

40 Strength and Defense would help a lot, then you could wear Rune Helm, Legs, and Shield. 43 Prayer would also help a lot because then it couldn't hit you with melee. That's about all you would need.

I'd go with my house, because if I respawned there I could go to 11 different places from it with all my portals and glory.

For an avatar it can be as big as 100x100, just size it to that size and add in what you want. To upload it you just go to edit your profile and there's a spot. Not sure about how big the siggy can be, but you need to upload that somewhere else like Imageshack.

Just pick the one that raises the skill you think most needs a boost.

This just sounds like a stupid scam to me. If someone tells you to hurt yourself, do you do it in real life? Well, maybe some people would, but I wouldn't, so I wouldn't do it in a game either
