All barrows set effects are random. dharoks is where your low on hp you hit higher, randomly!. Some peoples use this effect and keep their hp low enough in a duel/pk, so that they hit higher more often, instead of full healing they heal partially.

the fourth is a unapealable black mark i guess, because it is not mentioned in the apeal an offense section. I got 4 warning messages and 3 apeals.
