Ugliest has to be snakeskin boots, I'll almost always opt for dragon or infinity boots even when snakeskin provide the best bonuses for what I'm doing.

 

Second ugliest is trimmed herblore cape.

Full helmets. They look so BIG compared to the older ones.

 

And the H# rune helmets. Looks like a trash can on a guy's head.

Untrimmed strength, granite helm, granite body, and the dragon plate skirt are probably the ugliest items in the game.

I'd say the Voting hats, why an earth they're so popular I'll never know, you look like an Elf when you wear them.

Animal masks also rank high on my list, they'd be better if they were full masks rather than the ugly half-mask things they are now.

I dislike spotty capes. I'm also not a fan of skillcapes in general, I really only like the color schemes of con/mining/smithing.

I honestly can't decide on a single ugliest item, so here are a few of, in my opinion, some of the ugliest worn items.

• Granite Body
  
• Fighter Torso
  
• That pure outfit (e.g. Canifis hat with ghostly top, dragonhide legs, granite maul, etc.)
  
• Dragon Plateskirt
  
• The Darth Fatguy "Special Suit"

I liked them before, but with some graphical update they look WAY too big for you.
