Accuracy
flowers are crush weapons, so they only have -50 accuracy what I was trying to say that it's impossible to have a defense that is so high you cannot be hit. doesn't mean it's impossible to have an attack bonus so low that it's impossible for you to land a hit. If accuracy rolls are calculated anything like damage like I theorized, you'll be unable to hit regardless of your level if you have an attack bonus of -64. depending on your level, the attack bonus needed before you can no longer hit could be lower. which more or less goes in line with muggi said. max hit is calculated as min(1.3 + lvl/10 + atk bonus/80 + lvl * atk bonus/640) if atk bonus = 64, then the equation simplifies to min(1.3 - 64/80) = 0 so like how max hit would go to 0 if you had a -64 strength bonus your max attack roll should go to 0, preventing you from hitting any damage at -64 atk.

Is there a special way to level Dungeoneering?
duo with a good teammate so far seems the best =D get lvl 50 dungeoneering so you can bind a platebody stock up on food as you raid the dungeons, but dont stop to fish/make armor unless your combat level is really low, a platebody, good strategies, and looted food will carry you through most bosses take the time to make some gatestone + vengeance + cure poison spell runes when you spawn. gatestones make alot of boss battles easier vengeance can easily account for 25%+ of the total damage done to a boss. as for boss strategies hmm. place gatestones all around the sagittare boss fight map. That way you can run to him faster. Pray mage that way he'll be inclined to use range attacks more often, instead of his magic attacks which can snare you. Place gatestones @ the lodestones during the stomp fight. that way if you are blocked off from the lodestone, you can still teleport in note that this doesnt work if there is a rock is EXACTLY next to the lodestone, but it still helps. lexicus runewright is EXTREMELY weak to slash, you'll hit alot more accurately with a spear's slash even though the slash bonus on a pro spear is only +55. Run around during the rammernaut fight, and teleport out the moment you feel you are in danger. He is the only boss i recommend maging. drop gatestones between gluttonous behemoths and their food. The moment you die, teleport, and you will prevent the behemoth from eating. there are alot of other awesome strategies too. make sure you know ALL of them

Accuracy
I do not believe there is a level where you will never get hit or always get hit. there are certain monsters that cannot hit for damage (chickens) but even the mighty jad can take damage from a rooster. Post bag 38 implies that runescape's accuracy system is like a dice roll system. in which case, regardless of your defense rating, there is always a chance that you will roll a 0 for you defence roll also i have a question about the -65 never hit i'd imagine attack/range/magic level plays a part in this as well. is it known that -65 is the absolute minimum b4 lever hit? or could -60 also result in a never hit circumstance.

Accuracy
pvp world dust devils + restore/combat potions might be your best bet. I have a theory about how accuracy is calculated if you care to hear. I believe that in addition to the dice roll that determines your damage you also roll an "attack dice" and your opponent rolls a "defense dice" the attack and defense rolls are calculated the same way damage is calculated and finally if the attack and defense rolls are equal, defender wins. too busy with dungeoneering data collection to bother with testing this though =D

The hardest boss in Runescape
Well I got 1hitted, by the riftspitter's special "explosive" attack.. (while using a gatestone to tele back after death, forgot the explosive things were still there, and it 1 hitted my 910 hp) Also from the "rammeraut, I've seen a lv 583 boss.. - He had a max (melee, and he can disable prayer) hit from 800+.. That's just insane! So these bosses really seem to be sometimes "need luck to survive". the riftsplitter's fireportal deals increasing damage the longer you stay in it. conversely, this means that if you get out of it right away, it should deal little to no damage at all. if your reflexes are good, it will rarely deal over 100 life points to you.

Dungeoneering Reward Bags (Gem/Coal)
You mention the poison totem. Would you happen to know whether these are 1 handed or not? Tbh, poison is never really that much of a problem for me, I'm missing out on why everyone seems to be going made for the totem... The only thing I could think of it being used for is Zamorak GWD where you get poisoned and it hits quite hard last I remember. Only been there once and it wasn't fun since I forgot antis. uh pyramid plunder anyone? =D

I suuure hope this means 5x 200k xp lamps for the five elite diaries + some xp lamps for the lumbridge hard diary I'll let you guess where I intend to put those xp lamps. If your thinking in spending them in dungeoneering then consider this heads up: the only way for dungeoneering to be usefull is through the rewards, therefore you will need tokens, which are 10% of the exp gaine INSIDE deamonheim, and since pretty much every reward bar the chaotic weapons/augury scroll require overtraining (considering you save the tokens from the beggining) it would be kind of a waste to spend it there. If it is in fact the diaries i'll be getting myself a few herblore levels  xp rate goes up with the # of floors you can access, and the indications seem to point at it being somewhat exponential totally worth it. Anyways money isnt an issue for me.

-
Is it worth Pietying every task?
an extra 100k-200k per task depending on prayer gear but yes 100k-200k isnt alot of money

Dungeoneering Reward Bags (Gem/Coal)
I personally have enough tokens to buy both a gem back and a coal bag so I don't think having enough tokens is a problem right now I just don't think anybody in their right mind would spend their precious tokens on those rewards until they have a bone crusher, poison totem, arcane stream, etc.

The hardest boss in Runescape
you really need to learn some strategies >_< this boss is meant to be maged, ala saradomin GWD whoever has agro runs in a circle if the person who has agro gets hit once, he should immediately gatestone out of the room and heal up the rammernaut will stare at the wall for a few seconds while the rest of the team sends magical spells at him. rinse, repeat he should NOT be getting in melee range of any of your players.

A new maximum level?
I wouldn't mind that. It would just be rather inconsistent, and a little arbitrary. I would definitely prefer that solution over simply raising the cap though.

Dungeoning Question
But it only works if at least one member of the party is still in the dungeon. If they all leave then you can't rejoin. Yeah I haven't played with a team. So it's impossible to rejoin a solo dungeon? impossible :)
-
Dungeoning Question
if you're in a team, ring -> open interface -> rejoin party button

buying a sol.
depends on how much you enjoy dungeoneering even if you wait to use penguins/tears of guthix to get your prestige up before dungeoneering for tokens, it's gonna take you a while. ask yourself if 30+ hours of dungeoneering is worth saving yourself ~100k-150k gp per hour of bursting. probably not if you hate dungeoneering probably is if you love it.
