Most efficient way to max
Crawlers are around 5k cannonballs and 1.1 effigies per hour with a steel titan only. You can expect around 20 effigies from 100k cannonballs, possibly more of course. I'd suggest slaying for charms first. Should be noted that provided you have at least all effigy combos open, and if you don't value your cash, crawlers are the most efficient way to gain xp. This changes of course if you lock effigy combos, but cooking/firemaking makes very little difference. (courtesy of xpx, not sure on his actual calculations but the conclusions are there)

TDs
Replace the unholy book with a defender, that should help with darklight. Replace Verac's helmet with a helmet of Neitiznot, too. Replace the rune crossbow with Karil's crossbow or a handcannon.

Quick question with strength bonus vs prayer bonus
You should generally go for the highest strength bonus you can get (but fury over strength amulet & defender over dfs), but always have enough prayer bonus to last a whole task. Don't pray berserker though as it's not worth the space loss (unless you value extremes very highly).

Dungeonsweepers (DGS) - Huge changes; read first post.
Provided you get a ranked keyer + wingman, floors are generally around 25 minutes even with unhooded/gorg 2h people.

Most efficient way to max
At the moment ;). I believe the OP hasn't tried dgs yet, so that may well change in the future. I think 93-99 farming has the potential to make you 100m or more, I'd use effigies on fishing too. Right now you need, of the effigy skills: Fishing/farming Mining/smithing Agility/crafting Woodcutting/fletching Summoning/runecrafting Herblore/hunter Construction/thieving Only cooking/firemaking is all done. Since you also need combat stats, cave crawlers are likely the best choice for a fairly long time, though you also need to finish off slayer. That's not taking into account money (e.g. you're willing to dump the 200m into it no question). This would be faster than actually skilling, though that doesn't count the time spent making money.

The Plan
I don't think a yak is very good for Castle Wars, more like steel titan for the melee defence/scrolls or a unicorn. I'm fairly sure healing scroll + bandage beats another 30 inventory spaces. But in general Wade's plan is the way to get rich, the other way is merchanting.

nex gear
Well, ccbow/fury/onyx (i)/ranger's boots/alerter/barrows gloves with full pernix + divine is pretty much the best you can get (row switch and claw specs and rigour + ruby bolts). Alternatively (cheaper) you can use Armadyl or Karil's/third-age range(especially third-age range hat) and maul on curses, or Armadyl helmet with rigour. Another way is using void deflector (pernix top ideally) with diamonds, switching to void top/melee helmet and maul+claws.

Frosties on a pure..worth it?
Well, assuming a pure has 80+ ranged, eagle eye and a cannon, I don't really see how your kills would be slower than on a main with the same stats. Void would of course add quite a lot to kph, so zerker pures do have an edge, but still.

nomad question
The only pot worth using is overload, imo. Though you should pot extreme attack + strength at the bank (replace with brews) and start the fight off with bgs or claw specs.

Killing green dragons
Green dragons are just like frost dragons except that bones are worth less and you can kill more per hour. But you can't actually yak more per hour, so you'll have to haul a ton of bones + hides by hand, too. Just put down a cannon in any random place with green dragons and kill. Use a low-risk setup (rapier, proselyte) and pray piety/turmoil.

Mobilising Armies
Quyneax is a her? Yes. Either way - you definately do get back more credit then you wagered whether you win or lose. If you can win, the credit boost is pretty significant but 4 minute loses are a sure fire way to get up your credits. Plus you want to get your rank up for the onyx ring - so why not kill two birds with one stone This. Hi mls :) Winning games do tend to give you more credits, so if you see an opportunity to win go for it. But that was always a good idea. I'd actually go for a dragonstone ring now that you're going back to MA.

Getting Overloads - efficient
3000 special recovers 1000 super antifires 2500 of each extreme pot 2500 overloads Leaves maybe 100k xp, make some extra extreme sets for Bork or so, or super prayers. http://www.tip.it/runescape/?page=herblore_planner.htm Some tools on the site are pretty nifty ;).

Frosts
I wouldn't use Verac's brassard - the skirt is excellent though. Try Guthan's platebody (usually the cheapest of the three) or borrow a Bandos chestplate. You set up the cannon and attack a dragon, be sure to bring enough food to last for one inventory + one terrorbird. Try to move the dragon over the cannon without losing attack turns.

Frosties on a pure..worth it?
Ranging frost dragons is really good until you get soul split. So yeah, ranging them would be worth it. If you are getting defence up and rigour, be sure to use void (requires 42 defence).

Ranging/Magic necessary for slayer max efficiency?
House portals are slower than teletabs. Housetabs cost more too.

Wielding Battleaxes and Longswords with two hands
This could work for some one-handed weapons, sure. Personally I'd rather see a balance of the basic weapons (bronze through dragon) to make the battleaxes, warhammers, 2hs, halberds etc. one speed faster (and also increase their dps, though perhaps not as much as the speed increase would show). That'd put the speeds at the same as their dungeoneering counterparts, which seems much fairer.

The Fight Caves
Should be noted that a) this guide is very out-of-date and b) using only deflect curses and vengeance for the last 60% health or so will kill Jad without the healers.

making cannonballs
The Jagex rules state: So that means you would be able to watch TV, maybe fold laundry or play other games on your PC - but vacuuming is almost certainly not allowed.

Training Defence?
If training with knives - remember that your xp/hr is only 2/3rds of what you would get on rapid. For shortbows, this is 75%, for 5 speed bows it's 80%, for 6 speed weapons (crossbows) 84% and so on. This is because the 1 speed difference between rapid and longrange gets smaller (percentually) as the weapon gets slower.
